We are seeking an exceptional Case Manager to join our team. We are a Case Management Agency that help individuals remain at home and avoid unnecessary or premature nursing home placement. We are currently looking to hire case managers in the area(s) of South Carolina:
To be considered for this position you must be or have one
of the following:
A Case Manager with a BA/BS and or MA/MS degree in a Health
or Human Services field from an accredited college or university, with a
minimum of two (2) years of assessment and care planning experience with
clients a must.
A Certified Case Manager with a minimum of two (2) years of
assessment and care planning experience with clients a must.
I am seeking to employ individuals in the following areas: Aiken, Anderson, Charleston, Columbia, Conway, Florence, Greenville, Greenwood, Orangeburg, Ridgeland, Rock Hill, Spartanburg, & Sumter.
